a) membership application b) religious exempt application c) site visit request form d) background clearance for owner/operator e) department of health clearance f) fire department inspection (no violations) g) cpr/first aid certification (2 staff members) h) DCF Childcare Transcript or college degree (A.A., B.A., B.S., M.A.,or higher degree) i) affidavit of good moral character (owner/operator) j) religious exempt determination documentation k) minimum health, safety, and sanitation standards checklist l) religious exempt annual statement
Religious Exempt Approval Process & Timeline (for New Programs)
1) Complete applications A, B, & C online and submit required fees. 2) Submit the required documents D-K to the headquarter office

3) Upon receipt of the application forms, fees, and required documents, the school will be placed on the schedule for a site visit. 4) Upon successful completion of the initial site visit inspection and demonstration of compliance with the Minimum Heath, Safety, and Sanitation Standards, the school will be granted a Religious Exempt Approval Certificate.
Note: The Religious Exempt Approval certificate must be granted and received before opening an exempt childcare facility.
Once you receive the religious exempt approval certificate is received, you must complete the Religious Exempt Annual Statement (no charge). The Religious Exempt Annual Statement must be submitted electronically to the state DCF.
