Behavioral Health Programs that provide Substance Abuse Treatment for Adults and Children and Education Programs including Child Care Centers, Before and After School Programs, Kindergarten through Grade 12 Schools, that service children ages birth to twenty-one should apply for membership, license exemption, and accreditation. Call the Accrediting Agency Office, if unsure which applies to you.
Providers with a Single Location or Multiple Locations that do NOT operate as a Franchise may apply for Traditional Program Accreditation.
Corporations that operate like a Franchise may apply for Corporate System Accreditation.
Payment Options
We accept payment by credit card, debit card, or check (invoice), Check invoice payment is due upon receipt. A NSF or return check fee will be added to any checks returned.

Fees are subject to change. Be sure to double-check the form(s) for the current fee schedule. All fees are no-nrefundable and nontransferable.
Cancellation Policy
The transaction may not be canceled once it is processed.
The Annual Membership Application form is to be completed by Programs applying for Traditional (single or multi-site locations) and Corporate Systems applying for Accreditation and/or Religious Exemption. It is a prerequisite to participating in the religious exempt program and accreditation.
The Religious Exempt Forms are to be completed by Programs applying for exemption from licensure. The Program must be an integral part of a church or parochial school.
Background Screening Information
The Accreditation Forms are to be completed by Programs applying for Traditional (single or multi-site locations) and Corporate Systems applying for accreditation and/or License Exemption.
Upon completion of the Self Study, the program shall submit a Self Study Review Request form. The Program may submit a request for Extension of Time to complete the Self Study, if necessary.

Do I have to complete an initial and renewal membership application?
Yes, membership is a prerequisite to participating in the religious exempt approval process and/or accreditation process.
Is membership renewed annually?
Yes, all programs must renew its membership annually.
